The Human Cost of Delivery Noise

Major projects rarely fail because leaders lack data, intelligence, or governance. More often, failure begins when critical information is filtered, softened, delayed, or reframed as it moves through the organization. In this provocative session, Charles Igwe explores the concept of Delivery Noise, the distortion of decision-relevant information caused by pressure, optimism, hierarchy, reputation risk, and compressed decision cycles. Drawing on lessons from megaprojects, aviation, healthcare, and high-reliability organizations, the session examines why dashboards can remain green while risk is quietly intensifying, why governance can sometimes reward compliance over clarity, and why relying on individual courage is not enough to protect critical decisions. Participants are introduced to practical mechanisms for strengthening decision integrity, including designated challenge, assumption testing, early-signal detection, and structured pattern recognition.
